A first appointment with an endocrinologist is really about getting to know your diabetes and figuring out the best path forward for managing it. They'll review your symptoms, medical history, and any risk factors to get a full picture of your health.

Here's what you can generally expect:

- A review of your blood sugar levels and A1c results
- Discussion of your current medications or insulin use
- A C-peptide test may be ordered to check how much insulin your body is still producing
- Recommendations for lifestyle changes like diet and physical activity
- A personalized diabetes management plan tailored to your needs

After the first visit, your endocrinologist becomes an ongoing partner in your care. They'll monitor your A1c over time, adjust your treatment plan as needed, and help connect you with other resources like a registered dietitian or diabetes educator.

@A DiabetesTeam Member I'm not a fan of fasting. Not only is it predictably uncomfortable but possibly dangerous because our bodies need the nutrients. Focusing on healthy food choices will help the body find it's equilibrium in all areas (weight, health, etc.).

For me it was a referral from my PCP and it was 4 months before my appointment. My PCP was out of her comfort zone.

After 5 months on oral prednisone to help heal my lungs my numbers were over 22.2(400 USA) every day. Even after discontinuing the steroids my numbers did not come down. So I needed help and asked for Jardiance, long acting insulin, and fast acting insulin.

I was doing Jardiance, injecting 90 units long acting insulin daily, injecting 105 units fast acting insulin daily. And my numbers were running 6.1(110 USA) to 8.9(160 USA) daily.

So the endocrinologist asked for lots of bloodwork including genetic testing and specific bloodwork I had never had before.

He made an appointment to see me in a month and said we've got to reduce your insulin loading.

So I thought about it.
Intermittent fasting could lower my fast acting loading by cutting meals.

So I fasted 19 hours and ate 2 meals over the next 5 hours. Then I went to 1 meal and a snack. Then 1 meal.

That cut my fast acting insulin to 35 units a day. I also cut my long acting insulin to 35 units a day. So already down to 70 units from 195 units insulin. And I cut out the 750 mg of metformin xr 2x a day that my endocrinologist prescribed.

By the time I saw my endocrinologist at the 1 month appointment I was down to 30 units long acting insulin and 25 units fast acting insulin daily.

He said He would not prescribe the Promised Mounjaro and asked me to cut the long acting to 25 units daily. He made my next appointment for 3 months.

I cut out the insulins in 3 weeks. And began dropping the Jardiance to half a dose daily.

My endocrinologist wants me to remain on Jardiance for the cardiovascular protection it provides. He said see you in a couple of months via the texts we exchanged via My Chart.

Interesting thanks for the information. I find it very hard to fast as I go very low. (2.8 etc.)
