@A DiabetesTeam Member simple answer is because you are eating more carbs then your compromised system can handle even with the help any medications you are taking

A chat with your Doctor about what post meal (post prandial) range you blood sugar target is will tell you where your doctor wants you (standard advice is no higher than 10.0), but if you are exceeding that you are eating too much of the wrong thing in your meal

The solution would be to reduce the portion size of the carbs in the meal OR increasing/adding additional medications to take care of the excess.

And I am assuming you are noting those numbers at "2 hours after taking your first bite" which is the time/measurement used to make clinical decisions.

If you are simply spiking that high in the first hour after eating that can be considered almost normal. It is the 2 hour mark that is relevant in determining whether it's too high or not.
