I asked for Jardiance, long acting insulin, and fast acting insulin.

A standard dose is 5 mg 1x a day. If you use more. My instructions were to start with 5 mg at night. If that didn't work adjust up 2 mg the next night.

I eat no carbs.

Keep adjusting every night until it starts to work. Personally, after 5 months of prednisone my insulin resistance is sky high. So I increased eventually to 45 units at night. It finally touched my numbers. After a few days, it still barely touched my numbers.

So I started at 30 at night and 20 in the morning. Then when I got to 45 units at night and 45 units in the morning. It finally brought my lowest numbers down to about 165.

I asked for fast acting insulin again.
I started at 5 units per meal. It barely touched my numbers.

Increased it slowly to 35 units per meal. My numbers slowly came down to 135 to 140 as lows before meals.

Then over some time my numbers have slowly come down. I'm firmly in the nondiabetic range, but I'm doing 195 units of insulin a day.

It is what it is.

After a few months my insulin resistance is getting slightly better. So I can occasionally skip a dose of mealtime insulin.

But I need the long acting insulin.

Insulin resistant means you will have to use more long acting insulin to have the same effect as someone with less insulin resistance.

If it's long acting insulin. 1-2x a day is good.
