Diabetes can cause. Bladder problems such as retention and incontinence. Diabetes causes damage to the nerves of the urinary system. Have you seen a urologist? Also there is a device that looks like a tampon, you insert it into the vagina and it presses on something and helps you not to leak.

I have big problems with urgency when I don't watch the caffeine. You get even get pop that has no caffeine. As well as coffee & tea. As a first line of "treatment", I'd suggest you cut caffeine entirely and see if that helps somewhat.

In what I have read diabetes can damage the nerves controlling the bladder and cause incontinence. I would see a urologist for help.
